What have I been sent?

You have been sent an information article to read.

The article explains the medical procedure that your healthcare team have suggested.

The information article will tell you about the procedure itself as well as the risks, benefits and what might happen if you decide not to go ahead.

How does it work?

The first step is to open the article on your digital device (for example, your phone, tablet or computer). You will need to answer some questions to prove your identity, to match you to the correct article.

Please take time to read this article and if there is anything you are not sure about, ask your healthcare team.

Think about how your life might be affected by this procedure, for example, your job or hobbies.

You will be able to read the information article as many times as you like. You can also make notes of any questions to ask your surgeon when you speak to them.

Who has my data?

Your hospital has sent your information to EIDO Healthcare to allow this service to work.

Your hospital has asked EIDO Healthcare to keep hold of your information for an agreed period of time. It will then be deleted.

Your information will only ever be used in relation to your medical procedure.

If you are not happy for EIDO Healthcare to store your data, please speak to your healthcare team.
